Cortico-striatal-thalamic network functional connectivity in hemiparkinsonism

Cortico-striatal-thalamic functional connectivity (FC) was assessed in chronically treated (t-PD) and drug-nave (n-PD) hemiparkinsonian patients.. N-PD patients experienced an increased basal ganglia reciprocal connectivity while the affected caudate nucleus and thalamus were hypoconnected with the ispilateral fronto-insular cortices. The enhanced subcortical FC was normalized, or inverted into hypoconnectivity, in t-PD patients, who also exhibited an increased FC of the striatal and thalamic regions with the sensorimotor and association posterior cortices. FC changes occur in early hemiparkinsonian patients, antecede the onset of motor symptoms on the opposite body side, and are modulated by levodopa possibly through an increased thalamic outflow.
