T1 mapping and derived values such as partition coefficient and extracellular volume (ECV) have been used to differentiate normal myocardium from myocardium with diffuse fibrosis. There is scant theoretical and experimental evidence to-date favoring one measure over the other. In this study, we systematically compare the sensitivity and precision of T1 mapping measures. Using error analysis and measurements from phantoms, we show that post-contrast myocardium T1 (T1 cm ) has a higher precision than derived values like ECV. Comparing T1 cm of normal volunteers with heart failure patients, we show that T1 cm has higher sensitivity to disease discrimination compared with ECV.
