MRI-Based Radiomic Analysis of Trabecular Bone Surrounding Total Hip Arthroplasty in Femoral Loosening and Asymptomatic Patients

Motivation: MRI-based radiomic analysis of trabecular bone may aid readers in pre-operative diagnosis of implant loosening.

Goal(s): To quantify and compare image texture features of the trabecular bone surrounding the implant of symptomatic and asymptomatic THA patients.

Approach: MRI-based radiomic features were calculated for the femoral trabecular bone in pre-operative susceptibility reduced (MAVRIC) MRIs of symptomatic patients with THA loosening and asymptomatic patients with THA.

Results: While trabecular bone texture did not differ between symptomatic and asymptomatic groups, significantly less homogeneity, greater disorder, and the least correlation in the medial proximal Gruen zone, Zone 7, suggest asymmetric remodeling throughout the periprosthetic surface.

Impact: Image texture analysis may aid in identifying trabecular bone health, which can indicate an individual’s risk of aseptic loosening and potential requirement for revision surgery.
