On the Effect of Fat Suppression via Chemically Selective Saturation (CHESS) Pulses on R 2 * Measurements in Patients with Transfusional Iron Overload

Fat is a confounder in R2* based liver iron assessment
as fat-water modulations in multi gradient echo imaging
eventually affect the T2* evaluation. A simple solution
could be the inclusion of fat suppression (FS) via
chemically selective saturation (CHESS). However,
decreasing T2* times as observed with increasing liver
iron correspond to spectral profiles which overlap with
the CHESS frequency band. This partial saturation
potentially alters R2* leading to a biased iron
evaluation. Here, the effect of CHESS FS on R2* is
measured in 65 transfusional iron overload patients. A
phenomenological model is presented to explain and
correct the observed changes.
