A randomized controlled trial of statins to reduce inflammation in vertebrobasilar dissecting aneurysms (VBDAs) using vessel wall MRI

Synopsis

Motivation: Vertebrobasilar dissecting aneurysms (VBDAs) are a significant cause of stroke. Wall enhancement on vessel wall magnetic resonance imaging (VW-MRI) is a marker of inflammation. But VW-MRI has never been used in VBDAs for monitoring treatment response.

Goal(s): To perform the first-ever prospective randomized controlled trial (the highest clinical evidence) on the effect of statins for changes in VBDAs wall enhancement.

Approach: 40 patients with VBDAs were randomized 1:1 to either atorvastatin 20mg daily or not taken drug. VW-MRI was used to monitor wall enhancement and morphology changes over 6 months.

Results: Statins decreased wall enhancement of VBDAs in a short term.

Impact: Our results suggest VWI-MRI has great potential in future image-guided clinical trials to investigate novel treatment of VBDAs.
