Glymphatic Alterations Mediate Cognitive Dysfunction in Relapsing-Remitting Multiple Sclerosis

Synopsis

Motivation: Identifying more MRI biomarkers is needed to better understand the pathological processes responsible for relapsing-remitting multiple sclerosis (RRMS)-related cognitive impairment (Co-I).

Goal(s): To investigate the association between choroid plexus volume (CPV) and cognition in RRMS and analyze whether it was mediated by the glymphatic function.

Approach: 60 MS patients and 44 healthy controls were enrolled. DTI-ALPS indexes were calculated. Correlations between DTI-ALPS and CPV and cognitive performance were analyzed.

Results: RRMS patients with Co-I showed lower ALPS-index and higher CP volume. ALPS-index mediated the effect of enlarged CP on cognitive function.

Impact: The impact of enlarged CP on cognitive function is mediated by glymphatic alterations, indicating a pathological mechanism for MS-related cognitive impairment.
